DESCRIPTION:On behalf of the Fredericksburg Regional Chamber of Commerce\, we invite all members and their families to come out to the Publix Pharmacy at Cosner's Corner on Saturday\, November 2nd from 10:00 a.m. to 4:00 p.m. for Flu Shots. Publix Pharmacy are pleased to provide Flu Shots for your family members who are 4 years and older. In order to properly serve our members\, Publix Pharmacy has requested the following documents to be faxed over to the pharmacy no later than 1:00 p.m. on this coming Thursday (10/31):\n\n \n\n	Vaccination Consent Form: Please clearly include patients' names\, date of birth\, and all other essential information listed on the form. In doing so\, only fill out one consent form per patient.\n\n \n\n***Publix Pharmacy can only process flu claims based on the accuracy of the information provided***\n\n \n\n	Your Prescription Insurance Card: This card should specifically have processing codes for pharmacy claims\, these cards will typically have Bin numbers and RX group numbers printed on them.\n\n \n\nIf you have any questions\, please do not hesitate to reach out to the Fredericksburg Regional Chamber of Commerce or Publix Pharmacy!\n\n \n\n***A $10 Publix gift card will be given to each patient who receives a Flu Shot at the Pharmacy***
