The Client Care Coordinator is responsible for: conducting service inquiries and care consultations with prospective clients and/or their families; providing continued support to our clients by addressing service/care related questions; conducting quality assurance calls and home visits; introducing care givers to clients, and, providing care support to clients’ assigned care givers. The client care coordinator will develop home helper/companion client plans of care. This individual will also schedule and conduct semi-annual quality assurance meetings, individual will attend community events as well as develop and maintain relationships with Company referral sources.
The candidate must be professional, detail oriented, and possess a good work ethic. Excellent communication and computer skills are required. The ideal candidate must exercise good judgment and be able to work independently and as part of a team. Candidate should live in the Fredericksburg area. Some college preferred and experience helpful. Must possess a valid Virginia driver’s license and have a reliable and insured vehicle. A criminal background check and drug test will be performed.
